NEW Post-Surgery PE Recovery

The Nuss is done.
Recover like it matters.

A metal bar is lifting your sternum, and the next weeks decide how well it holds. The complete, cited recovery plan for after Nuss surgery.

Four phases, from the hospital stay to return-to-sport. You progress on your surgeon's clearance, never the calendar.

After the operation

The surgery is done.
Now what?

Most people leave hospital with a pain plan and little else. The bar is in, the hard part is supposedly over, but the first weeks are exactly when a corrected chest is won or lost, and no one hands you a day-by-day plan for them.

You're scared to move and undo it
The bar is not anchored yet, and a wrong twist genuinely can shift it. Without knowing which movements are safe, people freeze and stiffen up. This shows you exactly how to move, log-roll, cough and sleep so the bar stays put.
Discharged with no rehab plan
Your surgeon fixed the structure brilliantly, then handed you a leaflet. What to actually do at home for the next twelve weeks is left to you. This is that missing plan, breathing, posture, walking, and when to load.
The pain is more than you expected
The bar presses on the nerves under your sternum, so early pain is real. Staying ahead of it, and breathing deeply despite it, is what prevents a chest infection. The pain and breathing chapters cover exactly how.
Worried the result won't last
Surgery moved the sternum forward; spend recovery hunched and guarded and you hide the very result you paid for in pain. The posture and strengthening phases are built to keep it.
No idea when you can train again
Walk now? Lift when? Bench ever? Run? Most advice is vague. You get a clear timeline and a phased gym + bodyweight program that starts the day your surgeon clears it, posterior chain first, pressing last.
You don't know the red flags
A click, a fever, sudden breathlessness, which are normal and which mean call the surgeon now? The complications chapter spells out what is expected and what is an emergency.
If any of that is you

The plan you should have
left hospital with.

Not generic post-op advice, a phase-by-phase recovery program written specifically for the Nuss procedure, with every claim cited so you and your surgical team can trust it.

Mihail Veleski, rear view showing posterior-chain musculature
What this program is

Not a leaflet.
A phase-by-phase plan.

The problem with most post-op advice is that it's a page of don'ts and a follow-up date. Nothing on how to breathe so your lungs stay clear, how to move so the bar stays put, or how to rebuild strength without undoing the repair.

This program runs the whole arc in order: re-inflate the lungs and protect the bar first, restore upright posture and movement next, rebuild strength, posterior chain and core before pressing, then return to full training and sport. The enhanced-recovery pathways that do exactly this measurably cut pain, opioid use and time to walking after the Nuss.

A structured four-phase sequence, not a list of restrictions
Bar-safe movement: log-rolling, coughing, lifting limits, sleep
Breathing work that prevents the most common post-op complication
A gym + bodyweight strength program that starts on clearance
The Recovery Roadmap

From surgery
to normal.

Recovery is a sequence, not a guess. Every stage has one job, and you only move on when your surgeon clears it, not when the calendar says so.

Prehab Before surgery

Go in fit and ready, practise the breathing, build a posture base, sort nutrition, and set up home for the first two weeks.

The hospital stay Days 0–5

Multimodal pain control, walking on day one, hourly breathing work and clot prevention, what to expect and how to make it count.

Phase 1 · Protect & breathe Weeks 0–2

Re-inflate the lungs and let the bar settle. Move as one unit, no twisting, no lifting, log-roll everywhere.

Phase 2 · Posture & mobility Weeks 2–6

Restore upright posture and pain-free thoracic movement before any loading, so the corrected chest looks corrected.

Phase 3 · Progressive strengthening Weeks 6–12

Rebuild in the right order on your surgeon's clearance, posterior chain and core first, pressing later. The gym + bodyweight program begins.

Phase 4 · Return to full activity Months 3–6+

Progress toward full training and most sport while protecting the bar for its full term. Collision sport only with explicit clearance.

Why recovery decides the result

Surgery corrects it.
Recovery keeps it.

Recover passively and two things go wrong: your lungs underinflate and the small air sacs collapse, and you guard, hunch and trade a corrected chest for a rounded one that hides it. Active, early, guided rehab is the fix for both, and the evidence on it is strong.

25–49%

of thoracic-surgery patients develop a pulmonary complication, with collapsed-lung atelectasis the usual first domino. Consistent breathing work is front-line prevention, and this program builds it into every day.

~37% → 14%

the drop in post-op pneumonia seen with structured deep-breathing protocols, with atelectasis falling similarly. Ten spirometer breaths an hour is not busywork, it is the highest-leverage habit of week one.

First 6 weeks

is when body image, self-esteem and confidence climb the most after pectus repair. That payoff is earned by staying active and upright in recovery, not guaranteed by the operation alone.

From the evidence

Enhanced-recovery (ERAS) pathways after the Nuss procedure cut length of stay, opioid use and time to first walk; structured breathing and physiotherapy after chest-wall surgery reduce pulmonary complications and speed recovery. Every figure in the program maps to a numbered reference you can check.

What's inside

The whole recovery,
start to finish.

Every chapter is written for after surgery and every claim carries a citation, so you can trust it, and so can the surgeon and physio you're working with.

Who wrote this

Built around your surgeon,
never around them.

My name is Mihail Veleski. I've spent over a decade helping people through every stage of pectus, including the ones who chose surgery and needed a clear, safe plan for everything that comes after the operating room.

This program doesn't compete with your surgical team. It organises the breathing, posture, pacing and progressive strengthening that protect your repair and keep the result, every recommendation cited, every limit deferring to your surgeon's clearance.

Is this the right
program for you?

It's for you if
You've had the Nuss
Or the Ravitch, or you are scheduled for one of them
The weeks nobody covers
A day-by-day plan for what the discharge leaflet leaves blank
You want the why
Reasons behind each step, not just "take it easy for a while"
Back to lifting, safely
In the right order, on a timeline that respects the hardware
Cited, and deferential
Every claim sourced, and always second to your own surgical team
It's not for you if
You want it to replace them
Nothing here overrides your surgeon or your physiotherapist
You want to rush it
There is no version of this that skips clearance to move faster
You haven't had surgery
Every week here is anchored to a recovery date you do not have yet

This program never overrides your surgeon or physiotherapist. Their instructions always come first.

“I came in skinny with a deep dip and ribs that flared out. Six months later I am 4.5 kg heavier, and the flare is the part people notice is gone.”

What people ask
before they begin.

The four that come up most, answered the same way I would answer them on a call.

Take the 60-second quiz
How long before I see a change? +

Posture and breathing shift first - most people feel that inside the first few weeks. Visible chest change is slower. In the case log above, Milo saw his first real change at 2 months, Ian at 6, Liam at 9. The programs run 16 weeks because that is roughly how long it takes for the training to show up in the mirror.

Am I too old for this to work? +

Almost certainly not. The clients documented on this page were 22 to 32 when they started. Cartilage does stiffen with age, so progress is slower the older you are - but slower is not the same as stopped. What changes is the timeline, not whether it is worth doing.

Do I need a gym, or can I do this at home? +

Either works. Every program ships with a gym track and a no-equipment track for the same 16 weeks, so you pick the one that matches what you actually have access to. You can switch between them mid-program without losing your place.

Pectus excavatum, carinatum, rib flare - is it the same plan? +

No, and that matters. A sunken chest and a protruding chest need close to opposite work, and rib flare often rides along with both. There is a separate program for each. If you are not sure which one you have, the 60-second quiz sorts it out and points you at the right starting place.

FAQ

Common questions.

Is this a substitute for my surgeon's advice? +

No, and it says so on every page. This program organises the breathing, posture, pacing and strengthening around your recovery, but your surgeon and physiotherapist always come first. Where their instructions differ from anything here, follow theirs. It's designed to make you a better-informed patient, not to replace your team.

I've already had my surgery, is it too late to start? +

Not at all. It's ideal to read before your operation, but the bulk of the program is the twelve-week-plus recovery that happens at home. Whatever week you're in, pick up from the matching phase and use everything from there forward.

When can I start lifting again? +

Resistance training usually begins around week six and only on your surgeon's clearance, building to full loading by about three months. The program lays out a phased gym and bodyweight track that rebuilds the posterior chain and core first and brings pressing in last, with clear "not yet" lists so you don't push too early.

Will this help with the pain? +

It explains the multimodal approach your team uses and how to get the most from it, staying ahead of pain with scheduled non-opioids, tapering opioids early, splinting a cough, and the breathing that prevents a chest infection. It is education, not a prescription; your pain medication comes from your surgical team.

What format is it in? +

It's a structured, written program you can read on any device, clear chapters and phases with tables and a day-by-day routine, so you can reference exactly what you need mid-recovery without scrubbing through video. Every factual claim links to a numbered reference.

Is it specific to the Nuss procedure? +

Yes. It is written around minimally invasive (Nuss) repair, with the open Ravitch repair covered too. The bar-safety, breathing and phased-strength guidance is specific to a chest that now has a bar settling against the ribs.

How is this different from free advice online? +

Free content is scattered, a breathing tip here, a restriction there, none of it sequenced or sourced. This is the whole recovery in order, written for after surgery, with every claim cited. The value is the system and the trustworthiness, not the individual facts.

What's your refund policy? +

30 days, no questions asked. Read the program, use it, and if it isn't what was described, email me for a full refund.
